Woman Making a Difference Award

Nominate a woman in your life making a difference who demonstrates

the values that Tessie’s Serenity SPA embraces:

1.     Personal health & total well-being

2.     Protecting the environment

3.     Volunteerism (for example: church, community, charitable organizations)

Tell us her story using one or all of these values.  Please be as specific and as detailed as possible.   All nominations must be typed or legibly printed using the attached form.  Attach a photograph of your nominee preferably one that depicts her story.

 
Eligibility Requirements for Nominees:

• Must be at least 18 years old
• Must reside in San Bernardino , Riverside , Orange or Los Angeles County ;
• Winners must be willing to be photographed.  Photos will be featured on our website and future marketing campaigns including but not limited to our future Woman Making a Difference Awards

Restrictions:

• Deadlines for submission:  Noon , May 25, 2008
• Winners must continue to embrace the Three Values of this Award for a minimum of six months from date of award presentation;  otherwise, Tessie’s Serenity SPA reserves the right to take the trophy and/or plaques and title previously awarded.

• All submissions will be reviewed by the management and consultant of Tessie’s Serenity SPA.  All decisions are final.
• Timing of prizes will be at the sole discretion of Tessie’s Serenity SPA and other prize providers.
• Prizes are not redeemable for cash and are not transferable.
• To honor Tessie’s mother, winners will be announced on June 9, 2008
• Current employees and their families of Tessie’s Serenity SPA are ineligible to participate.
• Tessie’s Serenity SPA reserves the right to not award prizes to winners for which inaccurate or untruthful application was submitted.

2^nd Prize:            $50 gift certificate towards any spa service of her choice
